TURNING THE TIDE: EMERGENCY DEPARTMENT TRANSFORMATION AT SOUTHEASTERN COMMUNITY HOSPITAL
OVERVIEW
When Core Clinical Partners entered a new Emergency Medicine partnership with a community hospital in the Southeastern U.S. in Spring 2025, the challenges were clear. The Emergency Department (ED) was strained and patients were frustrated. Within two weeks, Core’s Clinical Operations and Operations teams were onsite, conducting an in-depth assessment and building an action plan with targeted strategies in mind to help the hospital reset.
By May, recommendations were in the hands of a newly appointed ED Nursing Director. Core also recruited a new Medical Director from the local community, someone who not only knew the region but also had trained nearby. Together, these leaders, backed by Core’s hands-on support and proven toolkits, set the foundation for a transformation that was as rapid as it was sustainable.
THE CHALLENGE
Like many hospitals nationwide, this ED faced an uphill battle. Patients experienced long waits and inconsistent flow through the department, causing delays in care. Staff felt the strain, patient satisfaction reflected the frustration, and Press Ganey scores were stuck in the single digits.
At the same time, the hospital was transitioning leadership, bringing on both a new ED Director and a new Medical Director. It was a moment of both risk and opportunity: without the right alignment, progress could stall. With the right alignment, however, meaningful change could begin immediately.
THE SOLUTION
Core’s approach begins with listening. By pairing a deep operational assessment with early leadership alignment, Core was able to focus attention on the areas that mattered most.
Within the first three months:
- CoreCARES Toolkits were introduced, bringing structure to patient flow and bedside experience. Bedside Coaching, Leader Rounding, Immediate Bedding, and “Run the Board” practices quickly became part of the daily rhythm.
- Leadership Education sessions gave the Medical Director and Nurse Leader shared tools and a common language to lead change together.
- Demand-Capacity Modeling helped reshape how APP hours were deployed, aligning staffing to patient demand in real time.
- Physician Coverage Optimization shifted away from 24-hour shifts, allowing providers to perform at a higher level with less fatigue.
Importantly, Core’s Clinical Operations leaders didn’t just hand off a playbook—they stayed onsite, month after month, ensuring these practices became sustainable habits. With a new team, a new approach, alignment with hospital leadership and priorities, this team was unstoppable. And the results demonstrated this powerful partnership.
THE RESULT
The story that unfolded was one of measurable progress and renewed confidence.
- Patient Experience: By September, the hospital recorded a 77% satisfaction score for the doctor domain and reached the 76th percentile in likelihood to recommend—a historic high for the ED up from the 8th percentile in the prior year.
- Throughput: With patient flow streamlined and staffing realigned, delays eased and care delivery accelerated. Discharge length of stay dropped by one half hour, and LWBS fell from 3% to 0.9%
- Clinician Engagement: The combination of local leadership and Core’s ongoing support strengthened accountability and collaboration across the team. A highly engaged Medical Director played a key role, partnering closely with Core’s Clinical Operations and hospital leadership to rebuild trust, integrate workflows, and create a shared commitment to improvement.
For patients, it meant shorter waits and more attentive care. For staff, it meant working in an environment where their efforts translated into visible results. For leadership, it meant confidence that the ED was finally moving in the right direction.
THE TAKEAWAY
This hospital’s transformation illustrates the power of true partnership. Core didn’t just provide recommendations; we worked side by side with hospital leadership to build trust, strengthen alignement, and equip teams with the tools to succeed.
The result was not only improved metrics but renewed sense of pride in the ED. By combining hands-on support, proven toolkits, and a collaborative approach, Core helped this Southeastern communtiy hospital turn a struggling Emergency Department into a stronger, more patient-centered environment.
Core Clinical Partners doesn’t just implement changes. We partner with hospitals to rebuild confidence, align teams, and deliver measurable improvement.
